Rueben Hernandez is passionate about using community health data to influence positive shifts in healthcare and social care access. He is a member of Elevate Health’s community-clinical linkages team and collaborates with cross-functional teams to support Elevate Health’s quality improvement, decision-making, and data storytelling projects. His focus areas include data collection and analysis; building comprehensive reports and dashboards; and conveying quantitative and qualitative data to diverse audiences.

Rueben served for eight years in the United States Marine Corps. He came to Elevate Health from the Washington State Department of Health where he served as the Data Manager for the Care Connect Washington program. While at WSDH he played a vital role during the COVID-19 public health emergency, providing essential technical support to community care hubs using the CCS care coordination platform and the state's disease registry.

He lives in Olympia with his girlfriend.
