Better Health for the Whole Population
We weave together health systems data with public health and population health information to create an entire picture of a community and its care needs, ranging from childcare to transportation, behavioral healthcare access, reliable internet service, and economic opportunities. By working with the community to address each of these factors, we help our partners develop integrated solutions and improve whole-person health.
Better Health Outcomes
We believe that when you approach health care at the local level, every individual receives the quality health care they need, regardless of social or economic status, disabilities, race, values, or beliefs. This approach results in better health outcomes for the individual. These outcomes are data-driven, measurable and create positive ripples that impact the entire community.
Better Health Care Experiences
Most primary care providers receive compensation based on volume of care, not outcomes, leaving them less time with each patient. Elevate Health understands that better health care experiences are a two-way street. We coordinate referrals, provide support, and supply data that providers need to deliver high-quality, individualized care. In turn, patients are connected with necessary services, feel seen and heard, and achieve better outcomes.
We bring the community of providers together to increase efficiencies, reduce overlap and eliminate redundancies. We lower the costs of healthcare by addressing environmental factors that affect health and connect clients to preventative care. We incentivize providers for fostering whole-person health, not solely dispensing services.

From Our CEO
Every organization has a heartbeat. An energy source that fuels their work. A place to draw inspiration. For Elevate Health, that heartbeat emanates from the communities we serve.
We know that our work is grounded in our community – so we created a communal table. Every community organization has a seat at this table and an equal voice to express their needs, goals and hopes. This is where our partnerships are born and ideas are generated.
We work in partnership with community, governmental and health care stakeholders to address the core challenges of today’s inequitable health systems and practices. We create whole-person health models, expand health care training opportunities, find new ways to support behavioral health services and pursue innovative approaches to large scale crises like the COVID-19 pandemic.
We believe better health is the cornerstone of community and economic vitality. And I know first-hand that accessing the care you need can be a heartbreaking challenge. Like millions of people, I have struggled to navigate our broken health care system. Twenty years ago, I was living in the Bay area, a single mom with a special needs child. My days were spent driving my son, Preston, all over the city to see specialists - none of whom ever seemed to talk to one another. The bills piled up, frustrations grew and worst of all, no one seemed to know how to best heal my child. The worry and expense were incredible. Eventually I left my job - and the health insurance it provided - to focus on my son’s health. This disconnected cycle of care went on for years, and it wasn’t until Preston was almost 18 that he finally received the diagnosis and treatment he needed.
During every hour spent on hold with a specialist’s office, sitting in every traffic jam on the way to endless appointments, and every time I lay awake at night, a persistent thought kept tapping on my shoulder: if getting health care for my son was this difficult for me – a woman with two masters degrees, professional health industry expertise and time - how hard must it be for everyone else?
That question fuels my work to this day.
My story echoes the story of so many Americans, of so many people in Pierce County. This is why at Elevate Health we work so diligently to serve our community. And why we are here for the long haul.
We built our non-profit Accountable Communities of Health (ACH) model on sustainable leading business practices, taking into account current data trends and encompassing funding from OnePierce Community Resiliency Fund (OnePierce). In this manner, Elevate Health instigates profound healthcare transformation and closes health equity gaps. The result? Long term, improved health for all Pierce County residents.
