Together with our partners, Elevate Health is building a robust, connected Community Care Hub. The Hub is a Pierce County network that not only addresses traditional health needs, but also the critically important social needs that are essential for people to thrive.

At the heart of our efforts are community-based workers, who serve as trusted guides, helping people navigate and coordinate care within the health and social care systems. Elevate Health serves as a centralized referral point providing infrastructure and capacity-building support to community-based organizations in our network.
