Arms Around You recently joined the coalition of agencies supporting Connect Pierce. Managed by Elevate Health, Connect Pierce is a free service that matches individuals and families with community-based workers who provide personalized, one-on-one care coordination support to help access available health and social care resources.
Arms Around You is a nonprofit re-entry resource and referrals program that serves formerly incarcerated individuals coming out of correctional facilities, the homeless community, and victims of domestic violence and substance abuse.
“Arms Around You is truly blessed to walk alongside Elevate Health in this healing journey for our community,” said Avon Curtis, executive director of Arms Around You. “Expanding access to health and wellness is essential, and this partnership reflects our shared commitment to ensuring our community receives the care and support it deserves. We are grateful and excited to begin this work together.”
Elevate Health will provide funding and support for community-based workers (CBWs) on the Arms Around You staff. In turn, Arms Around You will accept care coordination referrals from Connect Pierce and their in-house CBWs will participate in specialized trainings and a CBW forum convened by Elevate Health.

Arms Around You is one of 28 organizations currently contracted with Elevate Health to support Connect Pierce. Other recently contracted care coordination partners include the Pediatrics Northwest, Lutheran Community Services, and Pierce County Alliance. Elevate Health also recently announced ten new resource navigation partners who have joined Connect Pierce.
