Elevate Health announces ten new community-based resource navigation partners for Connect Pierce

Elevate Health, a nonprofit collaborative dedicated to improving health and social care outcomes across Pierce County, recently announced the selection of ten organizations who will serve as community-based resource navigation partners for its Connect Pierce community care hub.

Connect Pierce helps individuals and families overcome barriers to essential health and social services. Through personalized, one-on-one support from community-based workers, Connect Pierce bridges the gap between people and the resources they need to thrive, including housing, food, healthcare, legal aid, transportation, childcare, employment, and education.

Connect Pierce’s newly selected resource navigation partners include:

  • Associated Ministries
  • Community Healthcare
  • Family Promise
  • Innovative Change Makers
  • Multicultural Child & Family Hope Center
  • St. Vincent de Paul
  • Tacoma Community House & REACH Center
  • Tacoma Slavic Association
  • Therapy Fund Foundation
  • United Way of Pierce County


“We are thrilled to welcome these outstanding organizations into Connect Pierce,” said Gena Morgan, Executive Director of Elevate Health. “These partners bring deep expertise, cultural understanding, and a strong commitment to serving our diverse communities. Together, we’re building a more connected and responsive system of care for Pierce County.”

Each organization will receive funding to support staff members who serve as community-based resource navigators — professionals who serve as connection points between a family or individual and the services and resources that support their health-related social needs. Research shows that people who work with resource navigators are twice as likely to access needed services, leading to better health outcomes and fewer emergency room visits. Connect Pierce’s resource navigation partners will complement the work of the program’s previously established coalition of community-based care coordination partners.

“Resource navigators have extensive knowledge of local services and can quickly provide crucial information and resources to individuals referred to Connect Pierce,” explained Heather Thein, Elevate Health’s Senior Manager of Community-Clinical Linkages. “These new partners will also play a critical role in identifying individuals who may need more intensive support and connecting them to Connect Pierce’s broader care coordination services.”
