The Community-Based Care Network
Recognizing that good health and a person’s basic needs are inextricably linked, we’ve developed a Community-Based Care Network to help traditionally underserved populations.
Our Community-Based Care Network manages care coordination services in partnership with health care providers and community organizations. The Community-Based Care Network is a centralized hub where clients, providers, and community-based organizations refer people to the appropriate care program or services based on their level of need. This care traffic control model allows Community-Based Care Network staff to assign people to our PathwaysProgram—administered by our partners.
Through this program, we help people remove barriers and obstacles tied to the social determinants of health while connecting them with multiple modes of care.
OnePierce Community Investment Initiative
OnePierce is the investment initiative of Elevate Health that supports equitable whole-person health in the region.
With the support of funders, OnePierce distributes loans, grants and innovative funds to support community organizations that provide whole-person health and bridge health equity gaps in Pierce County.
