Transforming Health Systems—for Payers, Providers, and Patients

The Care Continuum Network

Recognizing that good health and a person’s basic needs are inextricably linked, we’ve developed a Care Continuum Network (CCN) to help traditionally underserved populations.

Our CCN manages multiple care coordination services in partnership with health care providers and community organizations. The CCN is a centralized hub where clients, providers, and community-based organizations refer people to the appropriate care program or services based on their level of need. This care traffic control model allows CCN staff to assign people to one of three current care coordination programs—Community Health Action Teams, Health Homes, or PathwaysProgram—administered by our partners.

Through these programs, we help people remove barriers and obstacles tied to the social determinants of health while connecting them with multiple modes of care.

OnePierce Community Resiliency Fund

OnePierce Community Resiliency Fund (OnePierce) is the investment arm of Elevate Health.

With the support of funders, OnePierce distributes loans, grants and innovative funds to support community organizations that provide whole-person health and bridge health equity gaps in Pierce County.

Community Data Trust

By creating a Community Data Trust with our partners, we are bringing together multiple stores of data from across clinical, government, tribal and social arenas, to create a real time composite view of a person’s needs and progress.

At the same time, the data we amass and analyze will allow us to give decision makers the information they need to focus resources on community health.
