Care Continuum Network
Recognizing that good health and a person’s basic needs are inextricably linked, we employ the Care Continuum Network (CCN) to help vulnerable populations in Pierce County. The CCN serves as a centralized community hub, where individuals, providers, community partners and community-based organizations may refer for services.
This care traffic control model allows CCN staff to assign individuals to one of three care coordination programs — Community Health Action Teams, Health Homes or Elevate Health Pathways Program – administered by our community partners.
