We are interested in organizational partners (community programs that house and/or hire the personnel who work with individuals) or referral partners (community programs who have individuals who could benefit from the network). Given Elevate Health’s focus on health equity, we are especially interested in recruiting partners in service to rural communities and underserved populations, such as people of color, veterans, immigrants and women.
