Elevate Health and Asia Pacific Cultural Center announce service agreement

Elevate Health and Asia Pacific Cultural Center (APCC) recently entered into a two-year service agreement that will support access to quality health and wellness care for Asian, Native Hawaiian and Pacific Islander residents of Pierce County. In particular, the partnership will focus on community-based care coordination delivered by community-based workers through Elevate Health’s Connect Pierce community care hub.
Connect Pierce is a free service that matches individuals and families with community-based workers who provide personalized, one-on-one support to help access available health and social care resources. APCC is one of 19 Pierce County-based organizations currently contracted with Connect Pierce.
“We are excited to join the Connect Pierce network and bring our cultural and linguistic expertise and our connection to the AANHPI communities to a broader system of care.” said Lua Pritchard, Executive Director of APCC. “This partnership reflects our shared commitment to equity, dignity, and community voice.”
Elevate Health will provide funding and resources for four community-based workers and a case manager who work within APCC’s Business and Community Engagement Services Department. In turn, APCC will partner with Connect Pierce by continuing to provide community-based care coordination services, conducting outreach and engagements to inform community about these services, accepting hub referrals for patients who would be well served by APCC, and participating in select trainings and a community-based worker forum sponsored by Connect Pierce.
