Elevate Health announces five new care coordination partners have joined Connect Pierce

Elevate Health has added five new care coordination partners to the coalition of organizations supporting Connect Pierce, a community care hub managed by Elevate Health that connects individuals and families with community-based workers (CBWs) who provide free care coordination and resource navigation services. The five new partners are Pediatrics Northwest, Asia Pacific Cultural Center, Catholic Community Services, Lutheran Community Services, and Pierce County Alliance.
Elevate Health will fund CBWs who work for each of the partner organizations, expanding Connect Pierce’s reach and deepening its ability to serve Pierce County residents. The CBWs will accept referrals through Connect Pierce, participate in specialized trainings, and engage in a community-based worker forum hosted by Elevate Health.
“This expansion comes at a critical time,” said Zach Powers, Elevate Health director of communications. “As federal resources shrink, local solutions like Connect Pierce are becoming even more essential. Over the past five years, Connect Pierce has helped nearly 17,000 Pierce County residents access the care they need—often when they’ve had nowhere else to turn.”
With the addition of the five new organizations, there are now 19 organizations contracted with Elevate Health to provide care coordination services via Connect Pierce. Elevate Health also recently announced the addition of 10 new partners whose CBWs will provide resource navigation services via Connect Pierce
Connect Pierce is supported by Washington’s Medicaid Transformation Project (MPT 2.0), and is focused on serving individuals and families on Medicaid/Apple Health and other local populations who frequently experience barriers to health and social care services.
