Elevate Health and Pediatrics Northwest partner to boost care coordination in Pierce County

Pediatrics Northwest and Elevate Health recently entered into a two-year service agreement that will increase access to health and wellness services for residents of Pierce County. The initiative will emphasize care coordination provided by community-based workers through Elevate Health’s Connect Pierce community care hub.

Connect Pierce is a free service that matches individuals and families with community-based workers who provide personalized, one-on-one support to help access available health and social care resources. A leading pediatric care group in the Puget Sound region, Pediatrics Northwest is now one of 19 Pierce County-based organizations currently contracted with Connect Pierce. 

“We are proud to join the Connect Pierce network,” said Rachel Lettieri, director of care transformation at Pediatrics Northwest. “The organizations and community-based workers involved with Connect Pierce share our mission of improving the health of children and families, and we believe that care coordinators can play a vital role in improving community health.”

Elevate Health will provide resources to three community-based workers and a managing director who will work within Pediatrics Northwest’s care transformation team. In turn, Pediatrics Northwest will provide community-based care coordination services to clients referred through the Connect Pierce community care hub.
