Elevate Health and Lutheran Community Services Northwest announce coordinated care partnership

Lutheran Community Services Northwest (LCSNW) recently joined the coalition of agencies supporting Connect Pierce. Managed by Elevate Health, Connect Pierce is a free service that matches individuals and families with community-based workers who provide personalized, one-on-one care-coordination support to help access available health and social care resources. 

“At Lutheran Community Services we’re dedicated to serving — and walking alongside — the most vulnerable members of our community,” said Najib Nazhat, Director of LCSNW’s Greater Puget Sound District. “That’s why we’re proud to join this network of community-based workers providing free care coordination services.” 

Elevate Health will provide support for two community-based workers and a team supervisor who will operate out of LCSNW’s Aging & Independent Living offices. The Connect Pierce-supported community-based workers at LCSNW will deliver community-based care coordination services, accepting referrals from the Connect Pierce, and participate in specialized trainings and a community-based worker forum hosted by Elevate Health.

Lutheran Community Services Northwest is one of 19 organizations currently contracted with Elevate Health to support Connect Pierce. Other recently contracted partners include the Asia Pacific Cultural Center, Pediatrics Northwest, and the Puyallup Tribal Health Authority.
