Catholic Community Services joins the Connect Pierce community care hub

Catholic Community Services of Western Washington (CCSWW) recently joined the coalition of agencies supporting Connect Pierce. Managed by Elevate Health, Connect Pierce is a free service that matches individuals and families with community-based workers who provide personalized, one-on-one care-coordination support to help access available health and social care resources.
“Our mission at Catholic Community Services is to serve all people, especially the poor and most vulnerable, with compassion, love and respect,” said Peter Nazal, CCSWW vice president and director of long term care. “Elevate Health and their Connect Pierce team share this mission, and we share their commitment to community-based health services.”
Elevate Health will provide support for two community-based workers and a team supervisor who will operate out of Catholic Community Services of Western Washington’s (CCSWW) Pierce County offices. The Connect Pierce-supported community-based workers at CCSWW will deliver community-based care coordination services, accepting referrals from the Connect Pierce, and participate in specialized trainings and a community-based worker forum hosted by Elevate Health.
“Catholic Community Services has long been a cornerstone of Pierce County’s social service landscape,” said Gena Morgan, Executive Director of Elevate Health. “Their deep-rooted commitment to meaningful community impact makes them an ideal partner for Connect Pierce, and we’re excited to collaborate with them in expanding access to care and support.”
Catholic Community Services of Western Washington is one of 19 organizations currently contracted with Elevate Health to support Connect Pierce. Other recently contracted partners include the Asia Pacific Cultural Center, Pediatrics Northwest, and the Puyallup Tribal Health Authority.

CCSWW IN PIERCE COUNTY: Catholic Community Services offers more than 25 programs across Pierce County, ranging from immigration legal assistance and family behavioral health to in-home care services. The organization also oversees several housing initiatives that provide short-term, transitional, and permanent housing solutions for individuals and families in need.
