Ash Martinez Recognized with the 2019 Community Health Worker of the Year Award

Congratulations to Ash Martinez, Community Health Worker with HopeSparks Family Support Services, who is the 2019 Elevate Health Community Health Worker of the Year Award winner. This award highlights a Community Health Worker who goes above and beyond their duties of service to the Pathways Community HUB program (HUB) ensuring equitable access to services for individuals in our community.
We are pleased to share Ash Martinez, Community Health Worker with HopeSparks Family Support Services, is the 2019 Elevate Health Community Health Worker of the Year Award winner. This award highlights a Community Health Worker (CHW) who goes above and beyond their duties of service to the Pathways Community HUB program (HUB) ensuring equitable access to services for individuals in our community.
In 2019, Ash demonstrated successful management of her clients as reflected in the quality of her work and consistent positive feedback from clients. This reflection of work yielded exceptional client satisfaction in our HUB program to support improved health and wellness for our community.
Additionally, Ash serves as a leader and representative to the CHW workforce. She continues to support other CHWs within the Elevate Health HUB providing additional training and technical support to her peer's growth and skills development. Ash presented at the Washington state Community Health Worker Conference this past Spring and represents Elevate Health HUB at the Pierce County Perinatal Mental Health Task Force. In all settings, Ash highlights the power of the HUB model and the positive impact CHWs make daily on the lives of individuals in our community.
Ash's dedication to make a positive impact in the lives of others is reflected in her accomplishments to date. We believe those qualities and her dedication to serving patient needs reflect the intent and value-add of the Elevate Health HUB program, the CHW workforce, and the community at large.
