Introduction

As Elevate Health is finalizing the Medicaid Transformation Project implementation plan for the region for a go-live date of January 1, 2019, we have developed a strategy for jump-starting the implementation activities with a goal of maximizing the impact of transformation activities on our project metrics. This opportunity is not designed to replace the January successful action plan implementation dollars for partners. Rather, this funding is intended to accelerate implementation of whole person integrated care. Elevate Health is looking to fund selected proposals designed to: a) move rapidly toward the achievement of level 5 or 6 on the SAMHSA integration scale; b) build capacity to successfully move to full value-based payment contracts; and c) pilot a health engagement teamthrough the Community HUB.

Opportunity

Elevate Health is looking to augment upfront internal investments for up to three healthcare delivery partners that are willing to equally invest into the goals outlined above. The opportunity is to receive a one-time infusion of up to $500,000 dollars in September to allow for dedicated project resources for accelerated implementation planning and action.
