Covid-19 Response Funding RFP
Washington State Department of Health (DOH) is extending funding for purposes of COVID-19 response through June 30, 2022.
In response to continued high COVID-19 infection rates, the Washington State Department of Health (DOH) is extending funding for purposes of COVID-19 response through June 30, 2022. This funding will support additional community-based health workers (CHW) who will conduct outreach and deliver care coordination support and interventions, and their supervisors, who will oversee the work of COVID-19 interventions in our community.

How to submit a proposal:
Please submit a Letter of Proposal outlining populations and areas of the county served by your organization, and infrastructure already in place at your organization to Sally@elevatehealth.org and include in your email subject line RFP CCWA2022.02.
