Join the Pathways Advisory Workgroup
We have new vacancies on the Pathways Advisory Workgroup and are in search of new members. The PAW is tasked with providing oversight and accountability to Pierce County residents. The Pathways model is unique in that services are coordinated through a single tracking system, allowing for the identification and elimination of duplicate services and the improvement of health outcomes across a defined service area and population.

The purpose of the PAW is to ensure standardized processes and to confirm all community stakeholders are involved and held accountable to help address whole person care. In addition, an advisory workgroup is a requirement to achieve national certification for the Pathways model, which we achieved in 2019.
We are looking to fill vacancies for the following:
- Substance Use Disorder (SUD) provider organization representative
- Referral partner agency representative
- Community-Based Organization representative
The workgroup meets quarterly at Pierce County Accountable Community of Health. The workgroup will
meet on designated Wednesdays from 2:00 pm- 3:30 pm. Meeting dates for 2020 are listed below:
- Wednesday, March 25, 2020
- Wednesday, June 24, 2020
- Wednesday, September 23, 2020
- Wednesday, December 9, 2020
Regular in-person attendance of workgroup members is required for the workgroup to function effectively.
If a workgroup member demonstrates continued absences from meetings, Pathways Community HUB Managers will
meet and discuss with the member directly.
If you and your organization are interested in participating, please contact pathways@elevatehealth.org.
