Introducing Community Care Conversations

Elevate Health’s vision is for every person we serve in our community to live a full, healthy, and vibrant life. We do this through listening to the needs of our community, providing information, creating relationships with our partners, and linking resources. In 2021, we are offering a forum of experts in our community to share information regarding topics that impact health and quality of life. We want this to be a conversation and your participation is important.

Held the third Wednesday of every month at 2:00 pm the series is free and open to the public. Topics that will be covered this year include:

  • Mental Health Crisis
  • Substance Use
  • Advanced Directives
  • Childhood Behavioral Health
  • Suicide Prevention
  • Understanding End of Life Care
  • Senior Behavioral Health
  • Aging in Place
